What you need to operate in Indiana
Indiana has no certificate-of-need review for clinics or imaging, registers every X-ray and mammography machine with the Department of Health, permits mobile dental facilities through the State Board of Dentistry, and requires an operator based outside Indiana to register a mobile health unit before it sees a patient.
Facility licensing for a mobile clinic
Indiana does not license a physician-run mobile clinic as a facility type of its own. The Indiana Department of Health’s Division of Acute and Continuing Care lists the provider types it licenses and certifies (hospitals, ambulatory surgery centers and others); a mobile clinic is not among them, so a hospital-run unit operates under the hospital’s license and a practice-run unit under its practitioners’ licenses. Confirm with that division before you assume no filing is needed.
One rule does reach the unit directly. If the operating entity is registered or licensed outside Indiana, 410 IAC 15.1, Mobile Health Care Entity applies: “A health care entity may not provide services in Indiana until the health care entity is registered with the department.” The registration covers mobile medical, dental and imaging services alike. An Indiana-based operator is outside this rule.
Certificate of need
Not applicable. The IDOH Certificate of Need Program, in effect since July 1, 2019 under Senate Enrolled Act 190, covers comprehensive care facilities (nursing homes) only. No review applies to a mobile clinic, a dental unit or imaging equipment.
X-ray and mammography registration
Any unit carrying an X-ray, dental X-ray or mammography machine registers it with the IDOH Division of Radiology, Weights and Measures, which publishes the Radiation Machine Registration Application and the inspection survey forms. Two of those forms matter here: “Mobile or Portable Radiographic” and “Mammography.” Register before the unit works in Indiana.
In every state, a unit that performs mammography must also hold a federal certificate under the Mammography Quality Standards Act before it examines its first patient. The FDA’s MQSA certification page sets out the order: accreditation by an FDA-approved body first, then the certificate, then annual inspection. In Indiana the FDA itself issues the certificate.
Mobile dental permits
A mobile dental facility or portable dental operation applies to the Indiana State Board of Dentistry through the Professional Licensing Agency’s dentistry licensing page. The page lists what goes with the online application: a $200 application fee; proof of a stationary location for record retention and communication for the mobile units; proof of radiographic equipment and of meeting the Department of Health’s safety rules for it (the registration above); and an example of the facility’s emergency care procedure.
Who does what. HLE supplies the vehicle side of the file: the floor plan, weights, power and, where the unit carries X-ray or mammography equipment, the shielding drawings the registrations ask for. The licenses, registrations and certificates above are held by you, the operator; we help you build the paperwork list before the build starts. Tell us the state and the program.
Checked Oct 15, 2026. Each statement above was read on the agency or statute page it links to, on that date. Rules change and local permits (county health, fire, zoning, parking) are not listed; confirm each item with the agency before you budget or schedule. Not legal advice.
Request a proposal
Tell us the vehicle. We tell you what it costs and what to attach.
Pick a plan or describe the program. Say whether you need a price, a bid package, a grant kit or a formal proposal. A person at HLE reads every request; nothing goes to a queue.
