RHTP: what state reviewers look for in a vehicle line

The Rural Health Transformation Program is CMS money paid to all 50 states, $10B a year for 2026–2030. It is not a mobile-medical program, but states can spend it on mobile units, and 26 states show a mobile-unit or vehicle signal in their plan, grant rounds or year-one awards. Here is how the money reaches a vehicle, and what the vehicle line has to survive.

What it is

  • $50B over FY2026–2030, announced Dec 29, 2025. Half is split equally among the states; half is based on rurality, state policy and expected impact.
  • Year-one awards ran $147M–$281M per state, about $200M on average.
  • CMS’s allowed uses: prevention, primary, maternal and behavioral health services; workforce; facility modernization and equipment; telehealth; EMS; care networks; payment reform. A mobile unit fits under services, equipment and EMS.

How the money flows

CMS pays the states. Each state runs its own grant rounds. Hospitals, FQHCs, health departments, EMS agencies, tribes and nonprofits apply, and the winners buy the vehicle.

Each year’s money has to be committed by the end of October and spent within the following year. That is why year-one rounds bunched up from March to September 2026, and why Year 2 rounds should start appearing from late 2026 into early 2027. Oklahoma’s Year 2 round opens in January 2027.

Whether a vehicle qualifies

Each state decides, and CMS reviews vehicle purchases case by case (Alaska’s guidance says so). It can go the other way: Kentucky’s mobile dental round excluded CDL-class vehicles, and Texas’s equipment awards excluded vehicles entirely.

Capital costs: Alaska caps them at 20% of the state’s award. Assume a similar ceiling in your state until it says otherwise.

Read the solicitation before you count on a vehicle. Some state details below came from page summaries rather than full solicitation documents; the tracker row for each round links the page it was checked against.

What state reviewers look for in the vehicle line

  • What the unit does that a building cannot. Alaska asks for exactly that. Name the sites, the days and the people served at each stop.
  • A vehicle line that fits the capital cap. If capital is capped at 20%, a unit that takes the whole capital share leaves nothing for the rest of the plan.
  • A budget that matches the documents attached: the plan sheet, the spec paragraphs and a range with the equipment itemized.
  • A way to buy it. Subgrantees spending these dollars follow federal purchasing rules: quotes under $350K, formal bids above. A unit specified around a published floor plan, with a budget range already in the application, is in the strongest position.

Stacking sources

RHTP or an HRSA grant often covers the program; USDA Community Facilities or opioid settlement money can cover the vehicle. The floor plan and the budget range are what tie the sources together.

Common mistakes

  • Assuming the vehicle qualifies because the state plan names mobile units. The round’s own rules decide.
  • Ignoring the capital cap.
  • Specifying a CDL-class unit for a round that excludes them, as Kentucky’s did.
  • The three mistakes every family shares: over-specifying, under-budgeting delivery, and missing the equipment line.

From HLE's Oct 7, 2026 review of Grants.gov and the state program pages. Where a source said "check the NOFO" or "not verified," this page says so too. HLE does not determine eligibility or write applications. We supply the vehicle and the documents about it.
